All-on-4 Recovery: Day-by-Day Timeline & What to Expect
All on 4 recovery is not a single event — it is a 3-to-4 month process from surgery to fully osseointegrated implants supporting a permanent zirconia bridge. Understanding what to expect at each stage helps you plan work, travel, meals, and daily life realistically, not optimistically.
This guide walks through what actually happens day by day, week by week, and month by month during All-on-4 recovery. I have written it from the chair side, based on what our international patients report at each check-in. If you want the timeline for how long to stay in Antalya during treatment, see our how long to stay guide. This article picks up where that one ends — after you fly home.
All-on-4 recovery in one sentence
Immediate discomfort for 2–3 days, near-normal function within 7–10 days, full soft-tissue healing by week 4, and complete osseointegration (implant fully fused to bone) by 3 months for the lower jaw or 4 months for the upper jaw. Then you return to Antalya for the permanent bridge.
Most patients underestimate the recovery ease and overestimate the recovery speed. In practice, you feel much better than you expected within the first week — but you cannot rush biology. The osseointegration timeline is not negotiable.
Day-by-day All-on-4 recovery timeline
Day 0 — Surgery day
Under local anaesthesia (with optional IV sedation), four implants are placed per arch — two straight in the front, two tilted at the back. A screw-retained temporary bridge is usually fitted the same day, so you leave with fixed teeth. Duration: 2–4 hours per arch.
What to expect this evening: local anaesthesia wears off after 2–4 hours. Mild-to-moderate discomfort, some bleeding on the gauze, and light swelling starts. Prescribed pain relief (paracetamol + ibuprofen combination usually) and antibiotics begin. Cold compress on the outside of the face in 20-minute intervals reduces swelling.
Day 1 — First morning after surgery
Peak swelling begins. Face may look “puffy” — this is normal and reaches maximum around day 2–3, then declines. Discomfort manageable with prescribed pain relief. You can talk, sip water, and eat soft cold foods (yogurt, ice cream, cold soup, mashed potatoes cooled). No hot foods, no straws (suction can disturb the surgical site), no smoking, no alcohol.
Day 2 — Peak swelling
Swelling and bruising most visible. Discomfort still present but easing. Continue cold compress and prescribed medication. Rinse with prescribed antimicrobial mouthwash (typically chlorhexidine 0.2%) starting today — gentle swishing, not vigorous.
Day 3 — Turning point
Most patients feel noticeably better today. Swelling begins to reduce. Bruising may still be visible for another 4–7 days but is only cosmetic. First clinic check-up (in Antalya) happens around this day — implant sites inspected, temporary bridge adjusted if needed.
Days 4–7 — First week complete
Swelling largely resolved by end of week 1. Bruising fading. Discomfort mostly gone — occasional twinges when chewing. Diet expands from purely soft cold to soft warm foods (well-cooked pasta, soft fish, scrambled eggs, soft-cooked vegetables). Continue mouthwash. Gentle brushing around the surgical sites with a soft toothbrush, avoiding direct pressure on implants.
Days 7–14 — Return home, soft healing phase
Most patients fly home around day 10–11 (see our how long to stay in Antalya guide for the full visit timeline). Soft tissue continues to heal around the implants and the temporary bridge. You return to normal work at your own pace — most desk-based professionals resume by end of week 2.
Weeks 2–4 — Soft tissue matures
Gum tissue heals around the temporary bridge. Comfort continues to improve. Diet expands further — you can now handle most soft foods, gentle chewing on the sides of the mouth where implants were placed. Still avoid: hard nuts, ice, crunchy raw vegetables (apples, carrots), tough meats, sticky foods (caramel, chewing gum).
Month 2 — Below-the-surface work
You feel almost normal externally. Internally, the crucial biological process — osseointegration — is well underway. Titanium implants are bonding with bone at the microscopic level. You may forget you had surgery, but the implants are still integrating.
Month 3 — Lower jaw ready
For lower jaw All-on-4 cases, osseointegration is typically complete by 3 months. You return to Antalya for Visit 2 (see our All-on-4 vs All-on-6 guide for the two-visit protocol overview). Visit 2 is 5–6 working days for the permanent zirconia bridge.
Month 4 — Upper jaw ready
For upper jaw All-on-4 cases, osseointegration takes about 4 months due to the softer upper jaw bone. Return visit scheduled around this time. If both jaws were treated, we plan the return visit around the slower-healing arch (usually the upper).
Post-Visit 2 — Permanent bridge fitted
Definitive Amann Girrbach zirconia bridge is bonded or screw-retained. You leave Antalya with your permanent teeth. Full oral function restored. Continue standard care indefinitely — brushing, flossing under the bridge with a superfloss or waterpik, and 6-monthly dental hygiene visits with your local dentist.
What to eat during All-on-4 recovery — week by week
| Week | What you CAN eat | What to AVOID |
|---|---|---|
| Days 0–3 | Cold liquids, yogurt, ice cream, cold smoothies, cold soup, mashed banana | Hot foods, straws, alcohol, smoking, chewing near surgical sites |
| Days 4–7 | Warm soft foods — scrambled eggs, soft pasta, mashed potato, soft-cooked fish, well-cooked vegetables | Hard, crunchy, sticky, or spicy foods |
| Weeks 2–4 | Most soft foods — well-cooked chicken, tender meats, cooked greens, ripe fruit (no skins) | Nuts, hard bread crusts, apples, carrots, tough meat, ice |
| Month 2+ | Most foods carefully — cut into small pieces, chew slowly on the sides | Ice, hard candy, fingernail-biting habits, opening packaging with teeth |
| After permanent bridge | Normal diet with common-sense restrictions | Ice, extremely hard objects — as with any dental work |
Warning signs — call the clinic immediately
All-on-4 recovery is typically uneventful. But contact us or your local dentist immediately if you experience any of these:
- Severe or worsening pain after day 3 — mild aching is normal; sharp, increasing pain is not
- Persistent heavy bleeding beyond 48 hours — light oozing on gauze is normal early on
- Fever above 38°C (100.4°F) — could indicate infection
- Foul taste or smell from the surgical site — sign of infection
- Numbness in the chin, lip, or tongue lasting beyond 48 hours — rare, but requires assessment
- Temporary bridge feels loose or dislodges — needs re-tightening promptly
- Any implant that feels mobile — call immediately
For patients who have flown home, we handle initial triage on WhatsApp and coordinate with your local dentist if in-person assessment is needed. This is part of the aftercare framework described in our is dental tourism in Turkey safe guide.
When can you fly home during All-on-4 recovery?
Medically, flying 48–72 hours after standard implant surgery is safe. Most patients fly home around day 10–11 after Visit 1 — this gives 2–3 days of buffer beyond the immediate post-op window.
If you had a sinus lift as part of your case, wait 7–10 days before flying. Cabin pressure changes can affect the healing sinus in the first week.
For the fully detailed flight guidance across all treatment types, see the “When is it medically safe to fly home?” section of our how long to stay guide.
Work, exercise, and daily life during All-on-4 recovery
- Desk work: resume day 3–5 as tolerated
- Physical work: resume week 2
- Light exercise (walking): resume day 3–5
- Moderate exercise (gym, cycling): resume week 2
- Contact sports: avoid until permanent bridge is fitted and cleared
- Swimming pool or sea: avoid until soft tissue is fully closed (2–3 weeks)
- Hot springs, hamam, sauna: avoid for first 2 weeks
- Alcohol: avoid for first 7–10 days, moderate thereafter
- Smoking: avoid entirely if possible — smoking significantly increases implant failure risk. If you cannot stop, cut down as much as possible
Long-term care after permanent bridge fitting
Once your permanent zirconia bridge is fitted, All-on-4 recovery is complete. Long-term maintenance is what determines how long the implants last (published survival: 94%+ at 10 years).
- Brush twice daily with a soft toothbrush, giving extra attention to where the bridge meets the gum
- Use a superfloss threader or waterpik to clean under the bridge — critical for preventing peri-implantitis
- Professional dental cleaning every 6 months at your local dentist
- Annual X-ray review of the implants (bone level check)
- Night guard if you grind — protects the prosthesis
- Avoid ice, fingernail biting, opening packaging with teeth
What if my All-on-4 recovery isn’t going smoothly?
Most cases proceed uneventfully. If something feels wrong, ask early — it is easier to address a small issue than a bigger one. Documented aftercare is part of the treatment plan; you should have written contact procedures and remote consultation availability from the clinic that treated you.
For patients who booked with us at Ulubey Dental, WhatsApp check-ins are scheduled at 30, 90, and 365 days — but you can message anytime with concerns, and we respond within a few hours.
Frequently Asked Questions
How long is the All-on-4 recovery?
Soft tissue heals within 2–4 weeks. Complete osseointegration takes 3 months for the lower jaw and 4 months for the upper jaw. Total time from surgery to permanent bridge fitting: 3–4 months.
How painful is All-on-4 recovery?
Most patients report mild-to-moderate discomfort for 2–3 days that is well controlled with over-the-counter or prescribed pain relief. By day 7 most patients need no pain medication. Peak swelling is at day 2–3, then declines. Patients typically say the anticipation was worse than the actual recovery.
Can I eat normally during All-on-4 recovery?
No — but the restriction relaxes weekly. Days 0–3 cold liquids only; days 4–7 warm soft foods; weeks 2–4 most soft foods; month 2+ carefully expanded diet; after permanent bridge fitting, normal diet with common-sense restrictions.
When can I fly home after All-on-4 surgery?
48–72 hours is medically safe for most patients. Most fly home around day 10–11 after the initial visit. After sinus lift specifically, wait 7–10 days.
Can I go back to work during All-on-4 recovery?
Desk-based work can resume within 3–5 days. Physical or manual work should wait 1–2 weeks. Most patients take 5–10 days off work total for the initial visit and immediate recovery.
Will I be able to smile during the 3–4 month recovery?
Yes. The screw-retained temporary bridge fitted during surgery is fixed and looks natural. Most patients say people cannot tell they had implants placed. You eat, speak, and smile normally with a soft-to-medium diet through the entire osseointegration period.
What are the warning signs of a problem during All-on-4 recovery?
Severe or worsening pain after day 3, persistent heavy bleeding beyond 48 hours, fever above 38°C, foul taste or smell from surgical sites, lasting numbness beyond 48 hours, temporary bridge loosening, or any implant feeling mobile. Contact your clinic immediately for any of these.
Can smoking affect my All-on-4 recovery?
Yes, significantly. Smoking reduces blood flow to the surgical site, slows soft tissue healing, and substantially increases the risk of implant failure during osseointegration. If you cannot stop entirely, cut down as much as possible — every reduction improves your odds.
